According to the Master Circular on Health Insurance Business 29052024 rolled out on May 29, 2024 by IRDAI, insurers are to decide on the request for cashless claims within 1 hour of receiving the request. Also, the final claim is to be granted within 3 hours after the policyholder’s discharge from the hospital. Insurers have been directed by IRDAI to establish necessary systems and procedures for this process by July 31, 2024.

The Insurance Regulatory and Development Authority of India (IRDAI) has a list of diseases/medical that are not covered under health insurance:
Any expense related to cosmetic surgery is not covered in the policy. These include Liposuction, Botox, or any similar kind of surgery. This is unless a doctor prescribes any plastic surgeries or hair transplants following a grave incident such as deformation caused in an accident or due to critical health conditions.
Most general health insurance providers also do not offer coverage for IVF procedures or fertility treatments.
Many health insurance plans in India do not cover medical expenses arising due to pregnancy and abortion.
Other charges such as registration charges, admission fees, and service charges related to medical treatment are not covered under certain health insurance plans.
Health supplement costs and protein shake expenses are not covered under health insurance in India. One has to pay these from their pocket.
Usually, health insurance plans do not cover Ayurvedic and Homoeopathic treatments. Treatments such as naturopathy, acupuncture, magnetic therapy, acupressure, etc. are not covered under the policy. Pre-existing diseases are not covered under health insurance right away. Certain plans do not provide cover for high blood pressure and diabetes, while many others do.
