What's Covered under the Cataract Care Plan?

Here are some of the inclusions under the Cataract Care Plan: 

Doctor Consultation or OPD Benefits 

The policyholder can receive a consultation with any doctor with a BDS, MBBS, BAMS, and BHMS degree up to the benefit amount that is mentioned under the plan details and specifications.

If the plan mentions any specific doctor or a medical specialist, you can consult with them. Otherwise, you can consult a doctor based on your choice. Its benefits are -

• Consultation with any doctor with a BDS, MBBS, BAMS, and BHMS (dental, homoeopathic, and ayurvedic dental) degree according to the specialists offered in the plan of your choice. Then you are required to submit an invoice for its reimbursement. It should clearly mention the doctor’s consultation fees, his/her speciality and name, his/her registration number, consultation date, the doctor’s stamp, and your reimbursement details.

• Every member mentioned under this policy can reap this benefit.

• You can claim this amount of benefit either in one visit or distributed over more than one visit for the doctor’s consultations. 

   

Lab Test Packages

This comprises a set pre-defined tests which are aimed at identifying and minimizing the risk factors in addition to the detection of illnesses at an early stage. You will have an option to undergo a check up for a pre-defined lab test package depending on the plan you chose, every policy year. The tests are available only at the prime partner hospitals of Bajaj Finserv Health and labs that get updated regularly.

What's not Covered

Let’s also understand some of the mandatory exclusions of the Cataract Care Plan – 

Lab and Radiology Test Package (some exclusions) 

• You cannot redeem the benefit against cash. 

• You can access this package only on the Prime Network of Bajaj Finserv Health.

• Your remaining benefit cannot be carried forward to the next year, even if you re-purchase or renew the policy after one year. 

• You cannot transfer this benefit to any other member.  

• You need to reap this benefit at one instance. 

• Only selected locations have home collection facilities available. In case your desired location is not available, you will have to visit the laboratories and take the tests. 

Network Discounts (some exclusions) 

• You cannot club the network discounts with any other benefit. 

• You cannot utilize the network discounts outside the Prime Network of Bajaj Finserv Health.

OPD/Doctor’s Consultation (some exclusions) 

• You cannot club the OPD/Doctor’s Consultation benefit with any other benefit. 

• You cannot redeem this benefit against cash. 

• If any medical procedure is undertaken, reimbursements will not be made. 

• Your remaining benefit cannot be carried forward to the next year, even if you re-purchase or renew the policy after one year. 

• You cannot transfer this benefit to any other member.

You can raise the claims for the the reimbursement for the consultation of doctors of the benefits of radiology or lab in these steps -

• Go to ‘Health Plans’ and select the plan you have purchased. 

• Tap on ‘Consultation of Doctor’ or ‘Radiology & Lab Benefit’ to select it, based on what you choose.

• Please fill in the details as asked and then upload the received invoice. 

• Share the details of your bank and then upload the cheque that is cancelled.

• Submit this claim. 

  

Now, within forty eight working hours, this claim amount will be credited directly into your bank account.

Bajaj Finserv Health App 

  

• Use your registered mobile number on the app of Bajaj Finserv Health to sign-in. 

• Go to your health plans section and select the plan you have purchased. 

• Tap on ‘Consultation of Doctor’ or ‘Radiology & Lab Benefit’ to select it, based on what you choose.

• Please fill in the details as asked and then upload the received invoice. 

• Share the details of your bank and then upload the cheque that is cancelled.

• You need to visit the services of healthcare offered by any laboratory or hospital under the Prime Network of Bajaj Finserv Health. 

• Give the billing executive the requested benefit on their network and your registered phone number. 

• Share the One Time Password (OTP) that you receive, with their executive. 

• After you give the OTP, you can get up to 10% discount on selected services or hospitals under the Prime Network of Bajaj Finserv Health.
