Here are the types of patients who are eligible to enjoy the benefits of the Ayushman Bharat Arogya Karnataka Scheme:
You are a patient and a resident of the state of Karnataka and belong to the ‘Eligible Household’ category as per the National Food Security Act of 2013
You are a patient and a resident of the state of Karnataka, but do not come under the category of ‘Eligible Household’ under the National Food Security Act of 2013.
You are also eligible under this scheme in case you do not produce the eligible household card. In this case, the treatment expenses will be on a co-payment basis.
For verification purposes, you need to submit some documents while applying for this scheme. Here is a list of the required paperwork:
Photocopy of your Aadhaar or ration card
Duly signed self-declaration
Duly signed consent form for Aadhaar-based validation
Here are details on the enrolment process for the Ayushman Bharat Arogya Karnataka Scheme:
The enrolment will be done on the basis of your Aadhaar card number
Your biometric impression will get captured on a biometric device, and it will be authenticated with the CIDR Aadhaar Server
After these steps, the e-KYC details will be auto-populated
If your biometric impression cannot be read, other options are provided, such as One Time Password (OTP), getting data from the QR code, or from the Food Department’s database
Under the Ayushman Bharat Arogya Karnataka Scheme, you get an enrolment card or the AB-Ark Card. In addition to that, you get a unique ID called the AB-ArK ID. You will get a health card called the Ayushman Bharat - Arogya Karnataka Card on successful enrolment.
The AB-ArK ID will serve as the PDS card number with a separator (-) as well as a sequential number. This will be applicable to every family member who approaches a PHI for service and gets enrolled. The card will contain basic details, such as your name, unique scheme ID, photo, etc.
You will get an SMS alert on your mobile number wherever the number gets shared with the registration personnel. Once the card is generated, you can get the treatment under the scheme. You only have to pay a one-time fee of ₹10 to get the card.
