1. Visit the official website of the Swasthya Sathi Scheme at https://swasthyasathi.gov.in/

2. Here, click on the option called ‘Apply Online’

3. A drop-down menu will appear. From the drop-down menu, click on the ‘Form B for Registration under Swasthya Sathi’ option

4. Download the form that appears and save a soft copy

 

You can then take a printout of the form to fill it.

Filling out the Swasthya Sathi registration form is a simple process. To make it easier and straightforward, here are the instructions you can follow to fill it:

1. First, fill in all the location details about your district, municipality, ward, and residential address. In case you are employed, you will also have to add your office address. 

2. Then on the top-right side of the form, you will have the option to enter your caste and minority status. 

3. In the given box, enter your name along with your father’s name. Below the box, choose the relevant option to state if any of your family members receive medical insurance or allowance from the government. 

4. Once the above details have been added, fill in the table with the list of your family members. Mention their details like gender, age, contact number, Aadhaar number, relation to beneficiary, etc. 

5. Once you have added the names of all your family and their details, you must add your signature at the bottom of the form in the designated area. 

6. Verify your information and submit your filled form at a nearby Duare Sarkar camp.
